Employee killed in fall through roof

Employee #1 and a coworker were removing and replacing deteriorated roof sheeting that consisted of three layers of 1 1/2 in. fiberglass sandwiched between corrugated sheets and steel decking. They had removed a 33 in. by 12 ft sheet of corrugated steel decking and were carrying it to the scrap pile when Employee #1, who was leading the way, started to walk across an area of exposed fiberglass insulation instead of directly up the newly replaced section. He fell 79 ft through the roof and landed on a metal frame inside the building. Employee #1 sustained massive blunt trauma and died.
