Two employees die in fall from boom lift

Employees #1 and #2 were on the platform of a Genie 580 boom-supported elevated work platform and were locating conduit at the top face of a 60 foot high building. The two employees had just elevated the first section of the boom to the lift and were approximately 35 feet high on the platform. The employees then rotated the lift 90 degrees toward the building and the machine fell over, away from the building. Both employees were thrown from the platform as the boom fell to the ground and hit a concrete mixer. Employee #1 died of blunt force trauma to the chest and Employee #2 died of head injuries. The failure to wear required ppe and lack of training for operation of a boom lift could be considered contributing factors.
