Investigation Summary
On March 31, 1997, Employee #1 and coworkers were helping to demolish a six-story building. Employee #1 and a coworker carried a large piece of concrete debris with wire mesh to the edge of a 6 ft wide by 120 ft long by 80 ft high floor opening. On the count of three, they released the debris. The wire mesh became caught on Employee #1's safety harness or lanyard and he went over the edge with it. He fell approximately 80 feet, landing on a large pile of demolished brick, mortar, and concrete. Employee #1 died of blunt force injuries to his head, chest, and abdomen. Employees were provided with and were wearing safety harnesses and lanyards with shock absorbers. Ropes in the general area were reportedly provided as lifelines, although they were not properly erected. Employees were not tied off or otherwise protected from falling.
