ELECTRIC SHOCK - DIRECT CONTACT WITH ENERGIZED PARTS

A maintenance millwright was working on a mill heat treating furnace. The 480-volt, three-phase electric heating elements were not working. After he diagnosed the problem as the contactor's control coil, he sought assistance from his supervisor. His supervisor instructed him to remove the coil and determine if anything was wrong with it. The millwright opened the disconnect for the contactor, but did not lock it out. He then removed the contactor. Another employee came upon the scene to show something to the supervisor. This employee closed the disconnect and then contacted the leads that had been removed from the contactor. He received an electric shock and was transported to a hospital. He died of his injuries on March 6, 1994.
