Employee's thumb tip amputated in press brake

Employee #1 was fabricating metal parts on an Amada press brake, adding various bends to the parts. He was holding a part in the point of operation and stepped on the pedal to cycle the machine. The part moved upward, trapping his left thumb against the brake body and amputating its tip. The machine was controlled by a single guarded pedal; the point of operation was unguarded.
