Investigation Summary
On June 9, 2017, an employee was washing the "corn husker" machine with a pressure washer using a short wand extension to clean the conveyor section of the machine. The employee climbed onto an active conveyor to clean with the short wand without completing lock out /tag out actions. The employee's hand was drawn into a nip point of the inbound rollers when it caught and drew in the wand tip into the rollers, partially amputating the employee's right ring finger and lacerating his right middle finger. The employee was hospitalized and treated for his injuries.
