Employee's finger broken by malfunctioning press

Employee #1 was operating a Bliss 28-ton full revolution mechanical power press equipped with barrier guard and two-hand control for point of operation function control. The press is used to punch a hole in the center of a metal dish-type disc that is 14 1/2 in. round. The part is inserted and removed by hand. Employee #1 placed the disc into the point of operation and cycled the press with the two-hand control. After the cycle completed, Employee #1 reached into the point of operation to remove the completed disc and the press tripped again, catching his right hand between the metal disc and the barrier guard. Employee #1 suffered a contusion and a broken right little finger. During the investigation, the power press was found to have a defective solenoid.
