Investigation Summary
At approximately 4:15 p.m. on June 30, 1995, Employee #1 was repairing a cracked dolly under trailer 7044, which was carrying a 42,340 lb load. A coworker was helping. The cab had been removed from the trailer. Because it was a rush job, they did not get another cab from the yard, but instead attached one side jack to the side of the trailer. No blocks or stands were put under the forward end of the trailer. They started to break loose the dolly to install a new one when the trailer started to shift away from the side jack. The coworker was in back of the spare tire mount and Employee #1 was in front. The trailer came down, crushing Employee #1 and bruising the coworker on the back and arm. 911 was called and other workers came to remove Employee #1. When the trailer was lifted, Employee #1 was found dead.
