Employee killed when struck by falling set-up fixture

At approximately 2:00 p.m. on December 18, 1992, Employee #1 was lifting a right angle set-up fixture onto a rotary table, using a pendant-controlled bridge crane and a single synthetic web sling. The set-up fixture slipped from the sling and struck Employee #1. Employee #1, who was transported to the hospital, treated, and admitted, died at 5:00 p.m.
