Investigation Summary
At 8:00 a.m. on May 31, 2017, an employee was unloading palletized materials using a rider electric pallet from inside a trailer to the dock. The dock plate was not secured correctly to the bed of the truck. The electric pallet jack got stuck between the dock plate and bed of truck. The pallet jack tire was jammed, the employee applied more power and the pallet jack recoiled. The employee lost footing and slipped and was crushed between dock door and pallet jack. The employee sustained a fractured foot.
