Investigation Summary
At 10:03 a.m. on June 5, 1992, at the Price Company, Rancho Cordova, CA, Employee #1 was stepping from one electric fork truck to another. He stepped on the hydraulic brake release while mounting the vehicle, and to gain leverage to pull himself up, grabbed the directional throttle lever, causing the second fork truck to move in reverse.
