EMPLOYEE INJURED WHEN FINGER AMPUTATED BY BELT PULLEY DRIVE

Employee #1 was servicing equipment with only portions of the equipment de-energized and not totally isolated from the power source. During the servicing of the equipment a short in the mode switch, used to disengage the section that Employee #1 was working on, caused the section to re-energize and start. Employee #1 suffered an amputation of a finger.
