Investigation Summary
At approximately 3:20 p.m. on February 21, 1998, Employee #1 of Foster Farms stuck his hand into a vertical auger to clean off debris. While his hand was in the auger, the equipment was inadvertently started. The equipment had not been de-energized and locked out. Employee #1's left arm was crushed, resulting in nerve damage. Employee #1 died as a result of his injuries.
