Employee's fingers amputated by guarded table saw

At approximately 9:00 a.m., at a cabinet shop, Employee #1 was preparing to cut plywood on a Delta RT31 table saw. He had been talking with someone before he walked over to the saw, and did not notice that the saw blade was running. When Employee #1 picked up the right side of the guard with his left hand to check the alignment, his fingers contacted the blade, which amputated his little and ring fingers to the second joint and his left middle fingertip. Employee #1 had 29 years experience and was trained. No violation was cited.
