Employee killed in 19 ft fall through roof opening

Employee #1, of Bernel Construction Inc., was on the roof of a building under construction installing eight 36 in. by 10 ft long skylights. This involved removing a roof sheet that was approximately 36 in. wide by 22 to 24 ft long, and replacing it with two short metal roof sheets and a skylight. Although coworkers on the roof were exposed to falling from a height of at least 7 1/2 ft, none were using safety belts and lanyards in conjunction with lifelines or catenary lines. While applying or preparing to apply mastic/sealant to the top edge of the opening, Employee #1 fell 19 ft through the opening onto a concrete floor, sustaining crushing cranial trauma. Employee #1 was transported to Central Valley Hospital, Hanford, CA where he was pronounced dead.
