Employee's fingertip amputated by air actuated valves

Employee #1, an untrained relief worker in the cheese department, was instructed to clean air activated valves with both 4 in. and 2 in. valve bodies at cheese vats #1 through #8. The working foreman states that he showed Employee #1 how similar valves had been cleaned. Having each completed eight 4 in. valves, Employee #1 and the foreman began working on CIP system 2 in. valves. They split up with Employee #1 working vats #2, #4, #6, and #8, and the foreman working vats #1, #3, #5, and #7. After he completed vats #2 and #4, Employee #1 attempted to install a valve plug assembly into the actuator, valve #253, on vat #6. When the air line was connected to the actuator, Employee #1's little finger became trapped between the valve plug and valve body. The application of air caused the air actuator to function immediately, causing the valve plug to retract into the actuator body. The movement of the valve plug assembly trapped his little finger, causing the partial amputation of the fingertip. He was moved for medical treatment to Hanford Medical Center, Hanford, CA, where he was treated and released.
