Investigation Summary
At 9:00 a.m. on June 7, 2017, an employee was using a 20 foot fixed ladder to access the roof of a building. While climbing the ladder, the employee lost his footing and fell about 6 feet to the ground. As a result, the employee sustained fractures to one of his legs and wrist. The employee was taken to a nearby hospital, where he was admitted for treatment.
