Investigation Summary
At 3:50 p.m. on May 23, 2017, an employee had cleared a jam in the Nissel ASB Machines #47 and reached in to retrieve a plastic bottle. The employee's right middle finger tip was amputated when the machine activated and moved the cylinder plate, catching his fingertip in a nip point. Hospitalization was not required.
