Employee killed in activated press, lockout not used

Employee #1, assisted by his supervisor, was changing a roll of paper in a Durand-Raute press. The supervisor exited the press, leaving Employee #1 inside. The press was not locked out and when the supervisor turned it on, a motor mount that secured its electric motor hit Employee #1. His head was pinned against a stationary object, causing massive injuries from which he died.
