Employee dies of head injury when knocked from roof by beam

Employee #1 was placing a coated steel beam, approximately 19 ft long by 8 in. diameter, onto a roof. No tag line was used but a single choker was utilized. Employee #1 was signaling by relay system to the crane operator while standing on the roof, approximately 12 ft above the ground, wearing no fall protection. As the beam was elevated over the roof, it started to slip from the choker and then spun around, hitting Employee #1 in the back of the head and knocking him from the roof to the ground. Employee #1 apparently died from a closed head injury.
