Investigation Summary
At 8:55 a.m. on May 23, 2017, an employee was off loading two 1,000 lb bags of gravel with a Moffett M55 forklift. The employee was hit by the roll-over protective structure of the forklift and fractured his arm as well suffering head trauma.
