Employee's hand amputated by molding machine

Employee #1 was operating a plastic injection molding machine with the safety glass removed. During the second shift he apparently leaned or fell into the machine's door. The closed door activated the machine, amputating the Employee #1's right hand. Only a foreman was present and management was not aware of Employee #1's actions.
