Investigation Summary
At 8:15 a.m. on May 26, 2017, Employee #1 and Coworker #1, employed by a metal fabrication company, were working at a multiemployer construction project for a manufacturing plant. Employee #1 was guiding a 5,000-pound valve frame that was mounted on the forks of a powered industrial vehicle operated by Coworker #1. The valve frame began to fall from the forks, and it struck Employee #1. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for a puncture wound on his right thigh and a laceration in the groin area of his right leg.
