Investigation Summary
At 8:30 a.m. on April 18, 2018, an employee was working to plumb the form work for the elevator shaft. His bag of pins was empty so he unhooked to climb down and fell to the ground, landing on his left side. The employee broke his left hip and left leg, which he was hospitalized and treated.
