Employee crushed in compactor and dies

On December 6, 1993, Employee #1, a second shift employee, was apparently making adjustments to or repairing a compactor when his head became lodged between the compactor and the machine frame. Employee #1 died. There were no witnesses. Interviews indicated that the timing device had malfunctioned about one week prior. The compactor was not locked out.
