Investigation Summary
A POWER LINE CREW WAS PERFORMING POWER LINE CONSTRUCTION WORK. THE CREW WAS REMOVING AN EXISTING UTILITY POLE. TO ACCOMPLISH THIS TASK, THE CREW CUT THE POLE INTO SECTIONS AND ALLOWED THE SECTIONS TO FALL TO THE GROUND. THE LAST SECTION WAS A 25 FEET, 9 INCHES LONG. IT FELL ON AN EMPLOYEE WHO HAD BEEN STANDING ON THE GROUND, BENEATH THE OPERATION, ALONGSIDE A DIGGER DERRICK. HE HAD MOVED TWO STEPS TO ONE SIDE OF THE TRUCK, INTO THE PATH OF THE FALLING POLE. THE POLE STRUCK THE EMPLOYEE IN THE HEAD AND KNOCKED HIM TO THE GROUND. HE DIED OF HIS INJURIES.
