Investigation Summary

Employee #1, who was working at a site where a four level concrete parking garage was being constructed, was climbing a ladder to join his fellow employees on their morning break. A Lorain 100 ton crawler crane was attempting to place a concrete bucket on a second level outrigger which was parallel to the ladder and about 6 ft away. When Employee #1 was near the top of the ladder, the spider gear in the boom hoist clutch assembly of the crane failed and the boom fell and came to rest upon the top level of the garage. When the boom landed on the garage, it shattered some form lumber, which rained onto Employee #1, causing him to fall approximately 10 ft from the ladder to the ground. Employee #1 broke two ribs and was hospitalized.
