Investigation Summary
Employee #1 was standing on a Sears, Roebuck and Co., type I extension ladder that had been separated to make two 14 ft ladders. He was attempting to secure an 8 ft section of 2 in. by 8 in. lumber to a warehouse loft joist which was approximately 12 ft 7 in. above a concrete floor. The employer stated that as Employee #1 tried to push the 2 by 8 up against the loft joist, the bottom of the ladder (the footing) slipped and the ladder kicked out. The employer stated that Employee #1 landed on his shoulder and head. Employee #1 was hospitalized and then transferred by air ambulance to a second hospital. The employer did not know whether he died during the transfer or while at the second hospital. The ladder(s) were set against the loft joist and were not secured. They did not have slip resistant feet. Employee #1 was acting within the scope of his normal job duties.
