Event: 11/09/1994
Employee dies in trench collapse

Employee #1 was installing a storm water drain line at the McCreary County Airport, Clark Hickman Air Field in Pine Knot, KY. He entered the unprotected trench to hold a surveyor's rod so that the employer could "shoot" a measurement with the transit. The trench was 2 feet wide and 9 1/2 feet deep. The trench collapsed, engulfing and killing Employee #1. The trench failure occurred in an area where the soil had been previously disturbed. Additionally, a Case 580 backhoe was close to the trench and was running. Employee #1 was performing his normal duties.
