Investigation Summary
At 10:45 p.m. on May 16, 2017, an employee was making a new product using a rubber mill. The employee cleaned the rollers by removing the excess product from the rollers. While the employee was removing the product from the rollers, a coworker switched the rubber mill on. The employee's left hand got pulled into the rollers as he was cleaning the rubber mill.
