Investigation Summary
At 6:30 a.m. on May 22, 2017, Employee #1 and Coworker #1, employed by a concrete company, were working at a multiemployer building construction project. Employee #1 was on his knees on the roof level (fourth floor) and manually hand troweling concrete between grid lines. Coworker #1 was operating a 9-horsepower power-driven Allen rotating trowel and manually guiding the trowel. The power trowel struck a dowel protruding above the concrete slab, which caused Coworker #1 to lose his grip on the trowel handles. The trowel rotated and struck Employee #1, who was approximately 6 feet away. Emergency services were called, and Employee #1 was transported to the hospital. He was admitted and treated for a deep laceration and dislocated right shoulder.
