Employee killed when run over by grass mower

Employee #1 was cutting grass using a Kubota LI85DT 15 hp tractor, pulling a Wood RM 500 mower. He fell off and was run over by the mower. The blade struck and cut him on the head and also cut a large gash in his back. Employee #1 died. There were no witnesses.
