Event: 12/23/1994

One employee injured, 1 killed, in tank explosion

At approximately 12:00 noon on December 23, 1994, Employee #1 and Employee #2 were cleaning a used 2,000 gallon tank with a steamer. The employees had cleaned the tank out four times (the norm is four to six times) before making a cut in the tank with an acetylene torch. Employee #1 was making the cut and Employee #2 had placed the steamer line in the tank, turned, and walked approximately 15 ft, when the tank blew up. Employee #1 was killed and Employee #2 suffered a broken arm and burns.
