Investigation Summary
Three employees were preparing to change rolls on a Valmet supercalender. Employee #1 was asked if she wished to observe the change and she indicated that she did because she would eventually be assigned this task. Employee #1 and two coworkers got on the threading (hydraulic lift) platform and rode to a height of approximately 5 ft. The crew leader on the floor was operating the 2 hoists to get them in position to lift the flyroll off its moorings, which are dovetails that rest on other sets of dovetails that are mounted on the cotton roll. Instead of leaving the securing bolts in place until the lift was ready to be made, the crewleader removed them. Apparently the cigarette holder (the lift tube used to lift the rolls at each end) on the right side of the roll came into a partial lifting position around the end, or journal. of the roll, lifted the flyroll from its mooring on the right side, and allowed it to fall on Employee #1, who was on the platform. She died of multiple internal injuries.
