Employee crushed to death under amusement ride

At approximately 10:45 p.m. on July 22, 1994, Employee #1 was operating an amusement ride called the "Tip Top" while the regular operator took a break. The ride's platform became stuck in a partially elevated position. The regular operator instructed Employee #1 to find the ride supervisor. Instead, Employee #1 reached into the pinch point between the frame of the amusement ride and the central bearing mount to free the linkage for the air cylinder. Employee #1 was crushed to death when the platform came down.
