At 10:00 a.m. on May 29, 2017, Employee #1 was on the floor, off-loading material from a truck trailer. While the employee was working, a forklift operator entered the area and struck the employee on his left foot. The employee sustained fractures and lacerations to the foot. The employee was hospitalized for his injuries.
