Investigation Summary

Investigation Nr: 787689
Event: 03/30/1993
Two employees injured in fall from beams

At 3:10 p.m. on March 29, 1993, Employees #1 and #2 were working at a height of 16 feet while attempting to move two sections of 3 foot by 14 1/2 foot metal grating. They had attached their harness lanyards to the grating in an attempt to slide it into position, but lost control of the grating while sliding it across the beams they were standing on. As the sections fell through an opening between the two beams, both employees were pulled after them. Employee #1 fell to the west, landing on some cross bracing of the structure. The sections of grating then fell on him, knocking his head against the cross bracing, breaking his jaw, and knocking out several teeth. Employee #2 fell to the south, falling over a 58 inch cement wall and landing on the cement and gravel below.
