Employee's foot/ankle crushed in log conveyor chain

At about 3 p.m. on February 22, 1994, Employee #1 was standing in the door of the sawyer's booth, talking to the sawyer. The sawyer then noticed that Employee #1 was not in the doorway but was walking toward the log stop/logfeed chain where there appeared to be a log hang-up. The sawyer turned back to his control console for the saws and sorter bins and almost immediately heard Employee #1 yell. The sawyer hit the kill button to lock out the chains and went to Employee #1. It appears that Employee #1 had tried to kick a hung-up log to get it loose but his foot was caught in the fast-moving log conveyor chain. His left foot and lower leg were pulled into the 1 3/8 in. gap under the log stop and between the chain. Employee #1 suffered a severely crushed foot/ankle and lower leg; the leg was attached to the calf by only a small strip of skin.
