Investigation Summary
At about 11:45 p.m. on May 11, 1994, Employee #1, an apprentice millwright at a sawmill in Joseph, OR, was sitting in the ring debarker control room talking to the operator. The operator started a log through the debarker, but bark dust accumulation had blocked the laser beam and the debarker jammed. The tail end of the log jumped to the side of the chain conveyor, which caused the outfeed hold down roll to drop prematurely and stop the log. The operator, thinking that he would take care of the jammed log later by mechanical means, turned to look at his incoming log deck and started sorting logs. Employee #1 left the control room, went down to the debarker, and kicked the jammed jog to align it in the chain conveyor. The knives on the ring debarker caught the log, flipping the tail end up in a circular motion and hitting him between the nose and forehead. Employee #1 was transported to Enterprise Hospital, then flown to St. Mary's Hospital in Walla Walla, WA, where he died at 12:30 p.m. after life support was terminated.
