Employee's head injured when rolled over by pipe

At approximately 6:40 a.m. on August 14, 1998, Employee #1 and a coworker were loading 40 ft long by 12 in. diameter pipes onto the forks of a forklift. To accomplish this, they built a ramp over a stack of angle iron. Two 12 in. pipes were banded together and stacked two high, so Employee #1 climbed on them to cut the band. The pipes rolled, and he fell between them. The second pipe rolled over his head, and Employee #1 sustained a massive head injury.
