Investigation Summary
Employee #1 was taking measurements from the top of a 23 ft high fiberglass storage tank. Before taking the measurements, the employee and an assistant tied off the bottom of the ladder he was using. As the employee ascended the ladder, the assistant stood on the bottom rung to keep it stable. After Employee #1 had descended three or four rungs, the ladder slipped on the fiberglass surface. The employee was thrown to the concrete floor. He struck his head and sustained massive head injuries. The employee was transported to Salem Memorial Hospital where he died 6 hours later.
