Investigation Summary
At 7:00 p.m. on May 30, 2017, an employee was attempting to move a heavy metal injection mold used for manufacturing plastic parts, when it fell off a shelf and struck him. The employee received critical injuries to his head and chest, and died on June 2, 2017. According to Medical Examiner, the cause of death was blunt force injury to the head.
