Employee fractures back in 12 ft fall

On November 5, 1992, Employee #1, a carpenter for Wildish Construction Co. in Tigard, OR, since September 15, 1992, was on the second floor of the Digester building at the Durham Waste Water Treatment Plant, installing door bucks in the concrete forms and reinforcing steel. This was being done in preparation for swinging in the outside gang form before pouring concrete. Placing the door bucks required a plumb line to be hung from the top of the forms for correct alignment. Employee #1 climbed the rebar frame, hung the plumb line, installed the door buck, and climbed the framework again to retrieve the line. On the second climb, Employee #1 grabbed an outside vertical rebar, which came loose, causing him to fall 12 ft onto a parapet wall. He suffered a fractured lumbar vertebra.
