Wood shaper machine severs employee's fingers

On August 23, 1993, Employee #1 was cleaning the surface of a wood shaper machine without isolating the energy source. As he wiped down the tabletop with a rag, it became entangled in the shaper head blade. Employee #1's right hand was pulled into the rotating head and her fingers were severed.
