Investigation Summary
Employee #1 operated a recycling truck by himself. He apparently had gone to the top of the truck near the plastic packer, a type of grinder that crushes plastic bottles. Employee #1 either fell into the packer or was attempting to push the plastic down and was caught and pulled into the packer. Employee #1 died of multiple severe injuries.
