Investigation Summary
At approximately 9:59 a.m. on February 10, 1994, Employee #1 was using a 32 ft fiberglass ladder to work on a fascia board on a townhouse. The base of the ladder had been set on the frozen ground in a manner meant to prevent movement. The work area was 90 degrees to the right of where the ladder was placed. Nailing movement caused Employee #1's ladder to shift, and he fell 28 ft from the ladder to grade. He struck the roof of a bay window before landing on the frozen ground. He was hospitalized for his injuries.
