Employee killed by falling scaffold frames

Employee #1 was preparing to transport metal scaffold frames (Waco, 6 ft 6 in. high) from a stockpile to the work area at the end of the building. He alighted the forklift and was setting the frames by hand onto the lift's forks. He set two frames onto the forks and as he turned to face the stockpile, it fell, in domino fashion, on top of him. Twelve to 14 scaffold frames fell and struck Employee #1 in the face and head. He died of his injuries. The metal scaffold frames weigh approximately 56 lb each. The scaffold frames were stockpiled in a near vertical position, thus creating an unstable material storage.
