Investigation Summary
At approximately 7:30 a.m. on November 11, 1993, Employee #1, a foreman, was operating a Sperry skid steer loader to unload a delivery of #4 steel rebar. The employee, who was working alone, was found by another employee in his crew trapped between the skid steer loader's upper control cross bar and the frame. Employee #1 died. No one witnessed the accident.
