At 6:00 p.m. on May 15, 2017, a mechanic was performing a changeover of powder resin on a machine. As he worked to change over the hoses on the machine, his glove became caught in an unguarded conveyor drive chain, that was located within 6 inches of the hose being changed. As a result, the employee's fingers was amputated.
