Investigation Summary
At approximately 1:30 p.m. on April 13, 1993, Employee #1 was in a trench checking the grade. The trench was 8 ft deep by approximately 15 ft long by about 5 to 6 ft wide at the top and 24 in wide at the bottom. Just as he started to exit the trench, a portion of the west side of the trench sloughed off and struck his lower legs. He was buried approximately halfway up to his knees. He was extricated from the trench in a few minutes by coworkers. Both bones in his lower right leg were fractured just above the ankle. The trench was not shored or adequately sloped.
